2021 Outstanding Community Outreach Honoree: Communities Lifting Communities


COMMUNITIES LIFTING COMMUNITIES

Communities Lifting Communities (CLC) is a community health improvement initiative led by the Hospital Association of Southern California (HASC) focused on reducing health disparities in Southern California. CLC is working to advance significant systems change through a collective impact model involving hospitals and health systems, public health departments, community clinics, Medi-Cal Managed Care Plans and community stakeholders.

CLC launched Cherished Futures for Black Moms & Babies, a collaborative effort comprised of CLC and the Public Health Alliance of Southern California, to reduce Black infant mortality and improve patient experiences and safety for Black mothers and birthing people in Los Angeles County.

CLC has done continued work in Diabetes Prevention and Food Insecurity Screening projects which include CLC’s Diabetes Prevention and Food Security Initiative in Ventura County, Kaiser Permanente and Providence hospital food recovery and redistribution projects, as well as Martin Luther King, Jr. Community Hospital’s “Recipe for Success” food program.

CLC hosts events such as Advancing Health Equity: Pathways for Hospitals to Improve Health Virtual Workshop which provides an overview of the role hospitals to advance health equity across their organizations and in the communities they serve.
