2023 Health Care Leadership Awards: EXCELLENCE IN IN-HOME HEALTH AWARD HONOREE

24 Hour Home Care is driving health equity in the Los Angeles area through an innovative new initiative, the Community Supports Home Care Program. It launched the program in 2022 to provide in-home care services to underserved communities in Los Angeles who might otherwise struggle to afford them.

The program is a direct reflection of the company’s mission: “to impact people’s lives by making a difference every day.” Within its first nine months, Community Supports served more than 100 underserved clients and provided nearly 30,000 care hours. Community Supports is on track to provide more than 200,000 total care hours to more than 200 people.

By partnering with health insurance plans, 24 Hour Home Care provides funded caregiving services to underserved residents, at-risk seniors, and the larger Medi-Cal community. Medi-Cal covers one out of every three Californians, more than half of school-aged children in California, half of California births, and two out of three patients in California long-term care. 24 Hour Home Care’s goal is to offer Medi-Cal residents a more equitable, coordinated, and person-centered approach to maximizing their health and life trajectory.

To support this mission, 24 Hour Home Care has secured contracts with six LA County Health plans for in-home services, including Kaiser Permanente Southern California, L.A. Care, Anthem, Blue Shield Blue Promise Health Plan, Molina Healthcare, and Health Net. These contracts cover and impact the lives of more than two million people in LA. Additionally, 24 Hour Home Care oversees 18 managed care organization contracts to enable clients to receive access to care through their benefits.
